Use the drop down selection box to locate your customer record.
If you cannot locate your record or you are not found within our system,
choose "Lookup Record" after choosing a department. You will be taken to a
blank form where complete information about the problem can be entered.
If any of the listed information is found to be erroneous,
please contact HR at (5140) to make corrections.
